Core Nursing Responsibilities
Clinical Care & Assessments
• Conduct comprehensive resident assessments and ongoing health monitoring
• Identify changes in residents’ physical, emotional, and cognitive conditions
• Develop, implement, and evaluate individualized care plans
• Perform wound care, catheter care, ostomy care, and other clinical procedures within scope
Medication Management
• Administer medications safely and accurately in accordance with physician orders and facility policies
• Monitor medication effectiveness and adverse reactions
• Maintain accurate medication documentation and MARs
• Collaborate with pharmacists and physicians regarding medication reviews and updates
Resident Support & Daily Care
• Assist residents with activities of daily living (ADLs), including mobility, hygiene, feeding, and toileting (as applicable)
• Promote dignity, comfort, and independence for all residents
• Provide emotional support to residents and reassurance to families
Care Coordination & Communication
• Communicate effectively with physicians, families, and interdisciplinary team members
• Participate in care conferences, shift handovers, and clinical rounds
• Escalate concerns promptly and appropriately when resident conditions change
Documentation & Compliance
• Maintain timely, accurate, and confidential health records
• Adhere to Ministry of Long-Term Care standards, CNO guidelines, and facility policies
• Support accreditation, audits, and quality improvement initiatives
Infection Prevention & Safety
• Follow strict infection prevention and control (IPAC) protocols
• Proper use of PPE and adherence to outbreak management procedures
• Promote a safe environment by identifying and reporting hazards
Supervision & Leadership (RNs/RPNs)
• Supervise and support PSWs and other care staff as required
• Provide guidance, delegation, and mentorship on shifts.
• Act as charge nurse when assigned.
